Residents and families of Chippewa and Mackinac Counties will benefit from a new community collaboration. MyMichigan Health has partnered with two health care organizations, Residential Home Health Michigan and Hospice of Michigan to continue home health and hospice nursing services for the two counties.
“When we learned the health department would be discontinuing home health and hospice nursing services, we began working together on a plan to make sure patients didn’t go without these resources. We were prepared to step-in; however, in our discovery, we learned it would be a lengthy process and would have left patients without needed services for a period of time,” said Rachel Aultman, M.S.H.L., B.S.N., R.N., A.C.M., system vice president of post-acute care, MyMichigan Health. “With our continued intention to serve our purpose in Creating Healthy Communities – Together, we reached out to partners serving this geography with the opportunity to fill these gaps. Residential Home Health Michigan and Hospice of Michigan both stepped in and agreed to provide these services. These two organizations are a perfect fit and this now gives patients more choices as to the home health care or hospice provider from which to choose.”
The partnership will give each organization strategic and operational support, and the opportunity for partners to share their best clinical practices and procedures.

From the very beginning they developed community-wide meetings discussing all aspects of post-acute care,” said Tracey Holt, executive director, Hospice of EUP, who operates Ball Hospice House, a five-bed, not-for-profit residential facility for patients receiving hospice care. “When we heard that the Chippewa County Health Department home health and hospice programs were shutting down, MyMichigan worked closely with us to develop options. MyMichigan was instrumental in partnering with us to bring Hospice of Michigan to Chippewa and Mackinac Counties.”

Residential Home Health is currently accepting new home care patients in Chippewa and Mackinac Counties. The Chippewa County Health Department will continue to serve existing hospice patients until June 30, 2024. Hospice of Michigan is expected to begin accepting new hospice patients this summer.
