Local health partners, including Aurora Health Care, Bellin Emplify Health, Public Health Delta & Menominee Counties, Marinette County Public Health, M&M Area Community Foundation, North Lakes Community Clinic, and Provident Health Foundation are urging residents to participate in a community health survey.
The Community Health Needs Assessment (CHNA) survey will provide valuable data for continued evaluation of community needs and help guide government, not-for-profit, and volunteer organizations as they create services to meet the most pressing health needs of the community.
In order for as many Marinette and Menominee County residents to have their voices heard, an online survey in English (Link: https://bit.ly/MM_Health) and Spanish (Link: https://bit.ly/MM_salud) will be available to anyone who lives in one of the two counties. This anonymous survey will take about 5 minutes to complete and is open to anyone 18 years of age and older.
The survey does not ask for identifying information, such as social security number, banking account numbers, or other personal identifiers. Questions focus on issues like access to primary care, health care insurance coverage, substance use, dental health, children’s health, mental health, chronic disease, and health behaviors. Results will be released to the public in early 2026.
The community health survey is created to:
- Gather local data to understand health outcomes and health needs
- Identify local health behaviors or risk factors that present opportunities for health promotion
- Track the success of community health improvementactivities
- Allow local health partners to document population health indicators to help guide health improvement efforts.
